Questionnaires: collect structured answers from patients
The problem it solves
Clinics, clinical trials and partners needed to ask patients things that forms and modules weren't built for: eligibility screening, feedback on a visit, a medical history. Those questions went out by email, paper or a separate survey tool, and the answers never made it back into the patient's record.
What's included
- Questionnaires as a task type. Assign them on their own or inside a bundle, block other tasks on them, and see completion alongside forms and modules.
- Multiple choice and free-form answers. Single-select questions for screening and scoring; long-form responses for anything a patient needs to explain in their own words.
- Build your own. Questionnaires are created and managed from the staff app for your clinic, with published and unpublished states, so you don't wait on EngagedMD to set them up.
- Shared module library. Practices can make selected modules available for patients to watch on their own from the resource library, with completions showing on the checklist and in the staff app.
